Dementia and drug use cause paranoia. That is very common. Some of other things that can include delusions and/or hallucination can be:
Alcohol
Mental disorder
PTSD
Bipolar disorder

Pharmaceutical drug (abuse or side effects)
Grief (yes, sadly)
